Network Recruiting Patient Experts

Northwest Renal Network (ESRD Network #16) is looking for motivated, committed and engaged patients to join various patient-centered groups called Learning Action Networks (LANs).  The patients’ role on the LAN would be as a Subject Matter Expert (SME).  These LANs will work with the Network on various projects focused on improving processes, outreach and outcomes in the Network region.  By participating on a LAN, patients will have the opportunity to have their voice heard while actively working to improve the quality of care that all patients receive.

If you know any patients that may be interested in applying to be an SME, please have them complete and return the SME Agreement form at http://www.nwrenalnetwork.org/P/SME/PatientSMEApp.pdf by February 15, 2013.  The majority of meetings will be held via conference call, and the Network will cover patients’ travel costs if there is an in-person meeting.  The Network will contact those patients who are accepted as SMEs.   Thank you for your assistance.  If you have any questions, please call Lisa Hall or Sharon Badger at 800-262-1514.
