Patient Engagement Progress

The latest issue of Network 16 News, the Network’s monthly newsletter for providers, has been published on our website, at .  In this issue we review our progress toward achieving the CMS goal and Network contract requirement to provide better care of the individual through patient- and family-centered care at the facility level.  We’ve created a metric for patient-centered care and patient engagement, and we’ve raised that metric by 10% since our 2014 measurement.  Still, the most common score in 2015 was 2 out of 13, so we’ve no shortage of work ahead of us.  To advance your own Patient Engagement program, use the tools at .
